Religious School registration form

Welcome new students!

We ask that this registration form be completed for all new students to Temple Sholom's religious school, whether they are students from families new to our congregation as well as any newly enrolling students from families with other children in school.

To submit the registration form, please download (using the link in top right), print, fill out and send back via US mail to the address at the bottom of the page - or you can scan the completed form and email to rs@sholomnj.org.
